You start with 3 Hearts and 1 Stamina wheel. There are 120 Shrines + 4 Heart containers from the Divine Beasts. You can also get 4 more spirit orbs from the Champion’s Ballad DLC. In total you can have a maxed health bar or stamina metre but not both. You can also duplicate hearts but that’s a whole other glitch.

it's because you used the save file that already had the maximum hearts/stamina. there is probably a separate value in the files that disables upgrading completely once you reached the maximum. just make a fresh save file and edit that instead. or just use the editor to manually give you the upgrade.
